RFID123 Video Manual:

Assembly Video:

PLEASE NOTE THAT THE SOLDERING INSTRUCTIONS FOR THE DIODES WERE NOT INCLUDED IN THIS ASSEMBLY VIDEO.  D1 AND D2 MUST BE ADDED.  Each diode has a white stripe on one side, and black on the other.  On the D1 and D2 footprints there are white stripes on the right holes.  Make sure that the white stripes on the diodes are facing the right before you solder them into place.  D1 and D2 are located right below the relays.  So the white stripe faces the right, and the black side of the diode faces left.  If you solder these in backwards, when the relay turns on, it will cause a reset. If you don't solder them in at all, you can damage the microcontroller.  It is very important.  If you have questions, let me know!
